Photography/Video Release/Messaging

Participants involved in any activities offered by Jacob Begin Basketball may be photographed or videotaped

during training. I hereby consent to the use of these photographs and/or videos without compensation, on the

Jacob Begin website, Instagram, Facebook or any promotional or advertising material. Initially below I

consent to receive text message updates from Jacob Begin Basketball via TextMagic messaging service.

Miscellaneous

This agreement contains the entire agreement between the parties and supersedes any prior written or oral

agreement between them concering the subject matter of this agreement. This agreement may not be waived,

altered, amended or repealed, in whole or in part, except in writing signed by both parties. This agreement

shall be governed by the laws of the State of Oregon, without respect to conflict of law provisions. Any dispute

concerning this agreement or any damages claimed by member shall be addressed by both parties.

Waiver

In consideration of the fee paid participant understands that there is a risk of personal injury in the course of

instruction and, with this knowledge, agrees to assume the risk of any injury and damages to participant

during the seminar/training/open gym participation. Specifically, participant agrees to hold harmless Jacob Begin Basketball and all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees and other participants connected with Jacob Begin Basketball from all losses, damages, injuries, causes of actions, claims, or complaints in the event that the participant is damaged or injured in any way during the participation, instruction and/or performance of any exercise or during any activity associated with the event location or during transit to or from the event. Participant further agrees to strictly obey instructors and observe safety rules. Because of the physical demands of physical fitness training participant understands that he/she must be in good physical condition to participate. Participant understands that in case of injury, the only medical treatment Jacob Begin Basketball will provide is first aid. Participant agrees that any pictures, audio, or visual recordings taken of him/her in connection with training can be used for publication, promotion, articles, shows and advertisement without additional consent and without compensation at this time or any other time. I have read and understand this release and agreement and agree to its provisions. I am not under their influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. I am over 18 years of age. (If not over 18 years of age, parent or guardian must agree to terms.)

 

YOU AS A PARTICIPANT HAVE READ AND FULLY AGREE TO THE TERMS OF THIS AGREEMENT. YOU ARE AGREE THIS AGREEMENT FREELY, VOLUNTARILY AND UNDER NO DURESS. YOU FURTHER CERTIFY THAT YOU ARE 18 YEARS OF AGE OR OLDER AND COMPETENT IN ALL RESPECTS TO ENTER THIS AGREEMENT. PARENT SIGNATURE REQUIRED FOR ATHLETES UNDER 18.
